This temporary position offers an opportunity to work directly with communities, providing critical TB and HIV outreach services while supporting patients throughout their treatment journey.

Key Responsibilities

As a Community Health Worker, you will play a frontline role in improving public health outcomes. Your duties will include:

TB & HIV Screening and Testing

  • Conduct TB screening in households and community settings.
  • Conduct TB testing and ensure results are received.
  • Facilitate linkage to treatment for individuals who test positive.

Patient Identification & Follow-Ups

  • Identify individuals who are HIV-positive or diagnosed with TB but not enrolled in care.
  • Follow up on missed appointments.
  • Conduct outreach activities and referrals to health facilities.

Education & Counselling

  • Provide accurate information about:
    • Living with TB and HIV
    • Preventing transmission
    • Benefits and challenges of treatment
    • Available healthcare services
  • Build trust and confidence in healthcare systems.
  • Support patients in self-advocacy and treatment participation.

Patient Support & Collaboration

  • Assist with enrolment or re-engagement in medical care.
  • Work alongside case managers and clinical staff.
  • Co-facilitate support groups.
  • Link patients to psychosocial support services.

Compliance & Administration

  • Ensure patient confidentiality.
  • Follow programme protocols.
  • Perform additional assigned duties.

Minimum Requirements

Education

  • Grade 12 (Matric)
  • HIV/AIDS Testing and Counselling Certificate

Advantageous Skills

  • Report writing
  • Computer and software proficiency

Experience

  • Minimum of 6 months related experience
    OR
  • 1 year related experience and/or training
    OR
  • Equivalent combination of education and experience
