Key Responsibilities
As a Community Health Worker, you will play a frontline role in improving public health outcomes. Your duties will include:
TB & HIV Screening and Testing
- Conduct TB screening in households and community settings.
- Conduct TB testing and ensure results are received.
- Facilitate linkage to treatment for individuals who test positive.
Patient Identification & Follow-Ups
- Identify individuals who are HIV-positive or diagnosed with TB but not enrolled in care.
- Follow up on missed appointments.
- Conduct outreach activities and referrals to health facilities.
Education & Counselling
- Provide accurate information about:
- Living with TB and HIV
- Preventing transmission
- Benefits and challenges of treatment
- Available healthcare services
- Build trust and confidence in healthcare systems.
- Support patients in self-advocacy and treatment participation.
Patient Support & Collaboration
- Assist with enrolment or re-engagement in medical care.
- Work alongside case managers and clinical staff.
- Co-facilitate support groups.
- Link patients to psychosocial support services.
Compliance & Administration
- Ensure patient confidentiality.
- Follow programme protocols.
- Perform additional assigned duties.
Minimum Requirements
Education
- Grade 12 (Matric)
- HIV/AIDS Testing and Counselling Certificate
Advantageous Skills
- Report writing
- Computer and software proficiency
Experience
- Minimum of 6 months related experience
OR - 1 year related experience and/or training
OR - Equivalent combination of education and experience
